Functional Disability Inventory (FDI)

The functional Disability Inventory is intended to measure limitations in activity in children and adolescents. It uses 15 questions to evaluate physical difficulty with activities.

Oswestry Disability Inventory (ODI)

The Oswestry Disability Inventory is intended to measure functioning. With 10 questions, it aims to assess how low back pain interferes with physical activity in daily living.

PedsQL

The PedsQL is a measure of health related quality of life, aimed at the pediatric population. Its results can be broken down into four subcategories.

Short Form 36 (SF-36)

The SF-36 is a questionnaire developed to look at physical, emotional and social well being. The findings can be further broken down into eight categories.

Roland-Morris Disability Questionnaire (RDQ)

The RDQ is a questionnaire that measures activity limitations due to back pain. It has 24 selections to be checked off in a yes/ no fashion. It is derived from the sickness impact profile (SIP)
